Cinacalcet is a medication used to treat secondary hyperparathyroidism (elevated parathyroid hormone levels) in patients with chronic kidney disease on dialysis, and to treat hypercalcemia in patients with parathyroid carcinoma or in those with primary hyperparathyroidism who cannot undergo parathyroidectomy. It works by directly lowering the levels of parathyroid hormone (PTH), calcium, and phosphorus in the blood, helping to control their levels.

How is this medication used?

Cinacalcet should be taken exactly as prescribed by your healthcare provider. It is typically taken once a day with food or shortly after a meal to increase absorption. Swallow the tablets whole; do not crush, chew, or break them.

What are the possible side effects?

Common side effects of cinacalcet include nausea, vomiting, diarrhea, and dizziness. Some people may experience muscle pain, weakness, or difficulty breathing, which should be reported to a healthcare provider. Very low calcium levels may occur and can be serious, leading to seizures, arrhythmias, or muscle spasms.

What are the potential interactions?

Cinacalcet can interact with other medications, including strong CYP3A4 inhibitors like ketoconazole, which can increase cinacalcet levels in the blood, potentially leading to adverse effects. Likewise, it can interact with drugs metabolized by CYP2D6 like flecainide or thioridazine. It's important to inform your healthcare provider of all medications you are taking, including over-the-counter drugs, supplements, and herbal products.
