Exelon (Rivastigmine) is a medication used to treat mild to moderate dementia related to Alzheimer's disease or Parkinson's disease. It works by inhibiting an enzyme that breaks down acetylcholine, a neurotransmitter involved in memory and cognitive function. How is this medication used?

Exelon is available in oral (capsule or solution) and transdermal patch forms. The starting dose for oral forms is usually 1.5 mg twice daily with meals, possibly increasing to a maximum of 6 mg twice daily based on tolerance and effectiveness. For the patch, initial therapy typically starts with a 4.6 mg/24 hours patch applied once daily, possibly increasing to a maximum of 13.3 mg/24 hours. The patch should be applied to clean, dry, hairless skin on the back, chest, or upper arm and rotated sites to avoid skin irritation.

What are the possible side effects?

Common side effects include nausea, vomiting, diarrhea, loss of appetite, weight loss, and muscle weakness. Some patients may also experience dizziness, headache, confusion, or increased sweating. Serious side effects can occur, such as heart issues, stomach ulcers, and seizures. It is important to monitor for signs of allergic reactions and report any unusual symptoms to a healthcare provider immediately.

What are the potential interactions?

Exelon can interact with anticholinergic medications, which can counteract its effects, as well as with NSAIDs (nonsteroidal anti-inflammatory drugs), which can increase the risk of stomach ulcers or bleeding. It may also interact with antidepressants, antifungal medications, and certain antibiotics. Alcohol can exacerbate its side effects, particularly dizziness and drowsiness.
